We are excited to share that the Community Health Equity Survey (CHES) / formerly COVID-19 Community Impact Survey (CCIS) is now live! Click here to access the survey.  The survey will be open from July 31, 2023, through early Fall 2023. Anyone aged 14 and older can take the survey.
